Homeless Assessments

By June 2021, Human Services will collaborate with community partners to complete initial assessments (Smart Path) for at least 1,600 individuals experiencing homelessness.

Smart Path to Housing and Health is the coordinated entry for people experiencing homelessness. Smart Path streamlines access to housing assistance through an assessment that prioritizes scare housing resources. Last fiscal year, HSD became the lead of Smart Path, although multiple partners conduct the actual assessments. HSD works collaboratively with partners who conduct assessments and shelter programs to locate individuals who have received a housing program referral through SP.
